There is a ready to use tool, at least in Linux.
You just configure /etc/smartd.conf; for example, I have:
/dev/sda -a -S on -T permissive -s (S/../.././02|L/../../6/03) -m root at telcontar.valinor
Which mails another local machine where I sit. Then you start the daemon:
systemctl enable --now smartd
